First Visit

At the first visit, you will have a private consultation with Dr. Allen on the information gathered at the consultation visit, your diagnosis and treatment options will be discussed with you, and a treatment plan made

In order to ensure you receive the maximum benefit from your insurance, one of our financial coordinators will discuss the coverage you are due, then will also help you schedule your procedure at a time convenient for you.

In order to make this process go smoothly for you, please bring the following to your initial visit.

1.    Your surgical referral slip and any x-rays if applicable.

2.    A list of your medications and your Rx bottles that your are taking.

3.   Your insurance cards and forms from both your dental and medical plans.

Important: All patients under the age of 18 years of age must be accompanied by a parent or guardian at all visits.

When you call to make your initial appointment, please alert the office if you have a medical condition (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever,) or if you currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If you dentist or physician has taken x-rays, you may request that they forward them to our office. If additional films necessary, they can be taken at our facility.
